Expert consensus on the long-term management of plaque psoriasis with biologic agents (2026 edition)

摘要: 【摘要】 银屑病是一种免疫介导的慢性、系统性、炎症性疾病,反复发作,长程治疗管理对控制病情、延缓疾病进展及预防共病发生具有重要意义。近年来,生物制剂已逐渐成为中重度斑块状银屑病长期治疗的重要系统治疗药物,并显示出良好的疗效。本共识基于近年来斑块状银屑病生物制剂长程治疗研究进展及国内外银屑病生物制剂治疗指南/共识,结合专家自身临床经验,针对斑块状银屑病生物制剂长程(48周至10年)治疗的疗效及维持方案、安全性、特殊人群管理(如老年人群)、共病管理等4大类共15个临床问题,形成23条推荐意见,为临床规范应用生物制剂长期控制斑块状银屑病提供参考,优化治疗策略、改善治疗结局。

Abstract: 【Abstract】 Psoriasis is an immune-mediated chronic, systemic, inflammatory disease characterized by recurrent episodes. Long-term disease management is essential for disease control, delaying disease progression, and preventing the development of comorbidities. In recent years, biologic agents have gradually become an important systemic treatment option for the long-term management of moderate-to-severe plaque psoriasis and have demonstrated favorable efficacy. Based on recent advances in research on the long-term use of biologic agents for plaque psoriasis, as well as national and international guidelines and consensus statements on biologic therapy for psoriasis, and incorporating the clinical experience of experts, this consensus addresses 15 clinical questions in four major domains, including the efficacy and maintenance strategies of long-term biologic therapy (48 weeks to 10 years), safety, management of special populations (e.g., older adults), and comorbidity management. A total of 23 recommendations were developed to provide guidance for the standardized long-term use of biologic agents in plaque psoriasis, with the aim of optimizing treatment strategies and improving clinical outcomes.
