摘要: 【摘要】 皮肤血管内皮细胞(EC)作为高度异质的血管功能单元,通过感知并整合免疫炎症微环境中的多维信号,在银屑病病理进程中发挥动态调控作用。皮肤EC在银屑病的发生发展过程中扮演双重角色:作为“感受器”,实时解析微环境中炎症因子、代谢产物等,驱动血管屏障功能障碍及免疫细胞浸润;作为“效应器”,EC通过分泌血管活性介质、重塑表观遗传及形成炎症记忆,主动参与辅助性T细胞17(Th17)免疫应答放大与组织炎症微环境维持。真皮乳头层毛细血管袢EC与免疫微环境的双向互作构成“血管-免疫”正反馈环路,并成为连接皮肤与系统免疫紊乱的关键枢纽。未来研究需突破传统“血管被动响应”范式,聚焦EC时空异质性对银屑病共病的调控网络,开发基于血管功能修复的新型治疗策略,为控制银屑病复发和共病提供新思路。

Abstract: 【Abstract】 As highly heterogeneous vascular functional units, skin vascular endothelial cells (ECs) play a dynamic regulatory role in the pathological process of psoriasis by sensing and integrating multidimensional signals in the immune-inflammatory microenvironment. Skin ECs play a dual role in the occurrence and development of psoriasis: as "sensors," they decode microenvironmental signals such as inflammatory factors and metabolites in real time, driving vascular barrier dysfunction and immune cell infiltration; as "effectors," ECs actively participate in amplifying T helper 17 (Th17) immune responses and maintaining the inflammatory tissue microenvironment by secreting vasoactive mediators, remodeling epigenetic profiles, and forming inflammatory memory. The bidirectional interaction between capillary loop ECs in the dermal papillary layer and the immune microenvironment constitutes a "vascular-immune" positive feedback loop, serving as a critical hub connecting skin and systemic immune dysregulation. Future research should transcend the traditional paradigm of "passive vascular response," focus on the regulatory role of EC spatiotemporal heterogeneity in psoriasis comorbidities, and develop novel therapeutic strategies based on vascular function restoration, offering new insights for controlling the recurrence of psoriasis and the occurrence of its comorbidities.
