Clinical and histopathological analysis of 173 cases of glomus tumors

Abstract: 【Abstract】 Objective To investigate the clinical manifestations and histopathological characteristics of glomus tumors. Methods A retrospective analysis was performed on clinical and histopathological data collected from 173 patients with pathologically confirmed glomus tumors at the Department of Dermatology, Xijing Hospital, Air Force Medical University from January 2002 to August 2019. Results Among the 173 patients, 125 were females (72.3%) and 48 were males (27.7%). The median age at the clinic visit was 39 years (range: 10 - 67 years). There were 107 cases (61.8%) with a disease duration of ≤ 5 years, and 66 cases (38.2%) with a disease duration of > 5 years. All 173 patients presented with solitary lesions measuring < 1 cm in diameter, which were located in the subungual regions (132 cases, 76.3%) or cutaneous sites (41 cases, 23.7%). The lesions were bluish?purple or purplish?red in 122 cases (70.5%), pink or light red in 33 cases(19.1%), skin?colored in 10 cases(5.8%), and black or grayish?brown in 8 cases(4.6%). Pain was reported by 168 patients (97.1%). Among the 132 patients with subungual lesions, 55 (41.7%) exhibited nail changes. Histopathologically, the lesions were classified as classic glomus tumors (148 cases, 85.5%), glomangiomas (21 cases, 12.1%), and glomangiomyomas (4 cases, 2.3%). Misdiagnosis occurred in 29 cases (16.8%) at the initial clinic visit. Among the 106 patients successfully followed up by telephone in August 2025, 9 (8.5%) experienced recurrence. Conclusions Glomus tumors predominantly affect females around 40 years of age and typically present as solitary, subungual, purplish?red or bluish?purple nodules characterized by sharp, stabbing pain upon pressure. Histopathologically, glomus tumors are categorized into classic glomus tumors, glomangiomas, and glomangiomyomas. Accurate diagnosis relies on clinical manifestations and histopathological findings.
