寻常型银屑病遗传流行病学分析

Analyses of Genetic Epidemiology of Psoriasis Vulgaris

摘要: 目的 探讨遗传因素在银屑病发病中的作用。方法 采用问卷和随访形式对720例寻常型银屑病患者及其家系进行遗传流行病学调查,应用Epiinfo6.0软件和Falconer公式对数据进行统计学处理和分析。结果 ①寻常型银屑病男、女患者的首次发病高峰年龄分别在20~29岁和10~19岁;男、女平均首发年龄分别为(27.57±12.18)岁和(23.01±12.40)岁(P<0.01)。②720例中有家族史者212例(29.4%)。男性有家族史者发病年龄早于无家族史者(P<0.01);父或/和母患银屑病的先证者首发年龄早于双亲正常的患者(P<0.05)。③Ⅰ型银屑病(早发型,首发年龄<40岁)各级亲属患病率均高于Ⅱ型银屑病(晚发型,首发年龄≥40岁)(P<0.01)。Ⅰ型中,女性平均首发年龄早于男性(P<0.01),Ⅱ型中无此差别。④先证者一、二级亲属的遗传度分别为71.07%±2.05%和36.77%±5.17%。结论 银屑病属于多基因遗传疾病,遗传因素在其发病中起重要作用。

Abstract: Objective To investigate the effects of genetic factors on the onset of psoriasis. Methods Seven hundred and twenty patients with psoriasis vulgaris were studied by questionnaire. Results ① For the male and female patients, the peak ages of the first onset were 20~ 29 and 10~ 19 years, respectively, and the average ages were 27.57±12.18 and 23.01±12.40 years, respectively.② In 720 psoriatics, 212 cases (29.4%) reported to have a family history of psoriasis. The age of onset was earlier in man with family history than that in man without family history (P<0.01),but the female patients did not; the age of onset was earlier in psoriatics whose fathers and/or mothers suffering from psoriasis than that in psoriatics with normal parents (P<0.05).③ The number of cases with typeⅠ psoriasis (early onset, age of the first onset< 40)was 633(87.9%),and that with typeⅡ (late onset, age of the first onset≥ 40) was 87(12.1%). The morbidities of first degree relatives were 5.77% in typeⅠ psoriatics and 3.45% in typeⅡ (P<0.01), and those of second degree relatives were 0.94% in typeⅠ psoriatics and 0.19% in typeⅡ (P<0.01).The onset of psoriasis was earlier in the females than that in the males in typeⅠ psoriasis (P<0.01), but it was not the same in typeⅡ (P>0.05).④ The heritabilities of psoriasis were 71.07%± 2.05% in first degree relatives and 36.77%±5.17% in second degree relatives. Conclusion The results show that psoriasis vulgaris may be a multigenic inheritable disease, and the genetic factors play an important role in the pathogenesis.
