大疱性类天疱疮与某些神经系统疾病间发病机制的研究

摘要: 目的 探讨大疱性类天疱疮(BP)与某些神经系统疾病(ND)之间可能存在的相关病理机制。 方法 分别以正常人皮肤组织和正常小鼠脑组织蛋白提取物作为抗原,以BP合并ND患者(12例)血清作为一抗,通过免疫印迹方法明确患者血清中抗体是否与脑组织发生反应。同时以单纯BP患者(42例)、单纯ND患者(15例)、正常人(30例)及抗BPAg1多抗作为对照。 结果 以正常人皮肤组织提取蛋白为底物,大部分BP合并ND组与单纯BP组患者可出现230 000、180 000或165 000抗原条带。BP合并ND组10例患者血清可识别脑组织中抗原条带(83%);单纯BP组患者中5例可识别脑组织中抗原条带(12%),单纯ND组患者中2例可识别脑组织中抗原条带(13%),正常人对照组无条带出现。3例合并脑出血的BP患者血清抗体可同时识别皮肤和脑组织中的BPAg1。 结论 BP患者血清中识别脑组织抗原的抗体可能在BP与神经系统疾病合并发生的机制中起到一定作用。

Abstract: Objective To investigate whether there is a possible pathogenic relationship between bullous pemphigoid (BP) and neurological disorders (ND). Methods The reactivity of the sera was assessed by immunoblotting method in 12 patients with BP and ND, 15 with ND only, 42 with BP only and 30 normal controls. The antigens were prepared with the extracts from normal human skin and mouse brain tissue. Results Most sera from patients with BP and ND or BP only could recognize the antigens of 230 000, 180 000 and 165 000 (Mr) in human skin. The antigens in mouse brain were recognized by the sera from 10 (83%) patients with BP and ND, 5 (12%) with BP only, and 2 (13%) with ND only. No reactivity was observed between the normal control sera and the antigens from human skin or mouse brain. The sera of 3 BP patients accompanied with cerebral hemorrhage could recognize the BPAg1, an antigen of 230 000 (Mr), in human skin and mouse brain at the same time. Conclusions These results suggest that both skin and brain can express BPAg1, which can be recognized by the sera of patients with BP and ND. The antibodies against brain tissue antigens in the sera of BP patients may play a role in the concurrence of BP and ND.
