氨基酮戊酸光动力疗法治疗尖锐湿疣对病灶乳头瘤病毒复制的影响

Inhibitory effect of 5-aminolevulinic acid photodynamic therapy on human papillomavirus replication in condyloma acuminatum lesions

摘要: 【摘要】 目的 检测氨基酮戊酸光动力方法(ALA-PDT)治疗前后,尖锐湿疣患者病灶局部人乳头瘤病毒(HPV)6/11型DNA载量的变化。 方法 选择电离子去除疣体后HPV6/11阳性患者分为两组,光动力组32例采用ALA-PDT治疗,对照组29例不治疗。3个月后用荧光定量PCR检测病灶局部HPV6/11型DNA载量。结果 3个月后光动力组29例中有26例HPV DNA阴转(89.7%),对照组19例中有13例HPV DNA阴转(68.4%);光动力组HPV DNA载量为(1.70 × 105 ± 7.86 × 105)拷贝/ml,对照组HPVDNA载量为(1.27 × 106 ± 2.21 × 106)拷贝/ml,两组比较,差异有统计学意义(P < 0.05)。 结论 ALA-PDT能降低尖锐湿疣患者病灶局部HPV病毒载量。

Department of Dermatology, Weifang People′s Hospital, Weifang 261041, Shandong, China Corresponding author: Zhao Xueliang, Email: zhxl5801@163.com 【Abstract】 Objective To detect the changes in DNA loads of human papillomavirus (HPV) type 6/11 in lesions of patients with condyloma accuminatum (CA) before and after 5-aminolevulinic acid photodynamic therapy (ALA-PDT). Methods After removal of warts by electrocautery, HPV6/11-positive patients with CA were divided into two groups: treatment group (32 cases) receiving two sessions of ALA-PDT with intervals of 7 - 10 days, and control group (29 cases) remaining untreated. Exfoliated cells were collected from the lesional sites of these patients before and 3 months after the first session of ALA-PDT, and fluorescence-based quantitative PCR was conducted to estimate the DNA loads of HPV 6/11 in these cells. Results Three months after the first session of ALA-PDT, 89.7% (26/29) of the patients in the treatment group and 68.4% (13/19) in the control group became HPV DNA negative, with significant differences between the treatment group and control group in the DNA loads of HPV ((1.70 ± 7.86) × 105 vs. (1.27 ± 2.21) × 106 copies/ml, P < 0.05). Conclusion ALA-PDT can reduce HPV DNA loads in lesions of CA in patients.
