强脉冲光联合5-氨基酮戊酸光动力疗法治疗痤疮

Intense pulsed light combined with 5-aminolevulinic acid-based photodynamic therapy for the treatment of acne

【摘要】 目的 将IPL技术和ALA-PDT相结合,评价其治疗炎症性痤疮的有效性和安全性。方法 41例患者随机分成治疗组21例和对照组20例。治疗组和对照组患者皮损区分别涂抹10% ALA霜或单纯霜。避光封包60min后,采用420nm~950nm IPL治疗。每周治疗1次,连续4次。治疗结束后第4、8、12周随访。结果 经过4次治疗和12周随访,ALA-IPL-PDT治疗组有效率达81.0%,平均疗效指数为75.2%;对照组有效率为35.0%,平均疗效指数51.0%。治疗组炎症性和非炎症性皮损平均减少率分别达到(83.6±4.1)%和(57.5±6.8)%。患者无色沉、水疱、结痂等严重不良反应。有3例治疗组患者出现一过性皮损加重和局部红肿。结论 ALA-IPL-PDT治疗能有效治疗中重度痤疮,效果维持达治疗结束后12周,且无严重的色素沉着等不良反应。ALA-IPL-PDT可作为临床中重度痤疮治疗方法选择之一。

Abstract:

【Abstract】Objective To evaluate the efficacy and safety of intensive pulsed light and topical 5-aminolevulinic acid photodynamic therapy in patients with inflammatory acne . Methods 41 patients with moderate to severe facial acne were randomly assigned to ALA-IPL-PDT group and IPL group. 10% topical ALA emulsions were applied on the faces of ALA-IPL-PDT group patients, while ALA free placeboes were applied on those of IPL group patients. After 60 minutes’ occlusion, all patients were treated with 420nm~950nm IPL. The patients in both groups had 4 treatment sessions at 1-week interval. 1 week after each treatment and 4,8,12 weeks after 4 treatments, efficacy and safety were evaluated. Results 12 weeks after 4 treatments, the effective rates of IPL-ALA-PDT group and IPL group were 81.0% and 35.0%, and the mean efficacy indexes were 75.2% and 51.0%. In IPL-ALA-PDT group, the mean clearance of inflammatory and non-inflammatory lesions was (83.6±4.1)% and (57.5±6.8)% respectively. There are no severe adverse effects like hyperpigmentation, blister and crusting. Conclusions The ALA-IPL-PDT is an effective and alternative treatment of moderate to severe inflammatory acne, associated with no hyperpigmentation and other severe adverse effects. The treatment effect is able to last for 12 weeks after treatments.
