点阵激光和针刺联合负压吸引治疗面部凹陷性瘢痕半脸对照研究

A half-face controlled study on the treatment of facial atrophic scars with fractional laser and pinprick combined with negative pressure suction

摘要: 【摘要】 目的 观察负压吸引联合点阵激光和针刺治疗面部凹陷性瘢痕的疗效及不良反应。方法 21例痤疮凹陷性瘢痕患者随机一侧面部行点阵激光和针刺治疗(对照组),另一侧面部在点阵激光和针刺治疗后联合负压吸引治疗(试验组)。统计第1次治疗后1、3个月及第2次治疗后1、3、6、12个月两组痤疮瘢痕严重程度评分(ECCA)、医生视觉评分(IVA),患者自评整体审美改善(GAIS)评分、皮肤病生活质量指数(DLQI)评分中位数及满意度评分中位数,统计不良反应及持续时间。采用SPSS27.0软件进行数据分析,组间比较采用配对样本秩和检验。各组各时间点间比较采用K个相关样本的Friedman双因素按秩ANOVA检验,两两比较结果采用Bonferroni法矫正。结果 第2次治疗后6个月,试验组ECCA评分[47.50(35.00,50.00)分]低于对照组[50.00(45.00,57.75)分,Z = -2.00,P = 0.045];试验组IVA评分[3.00(2.00,3.00)分]高于对照组[2.00(2.00,2.50)分,Z = -2.35,P = 0.019]。随着时间延长,两组ECCA、IVA评分分别呈现下降及上升趋势。治疗后1年随访,试验组患者满意度评分及GAIS评分均高于对照组(均P < 0.05);DLQI评分从治疗前6.0(3.0,8.5)分降至治疗后1.0(0.0,1.5)分(Z = -4.02,P<0.001)。两组的短期不良反应主要表现为结痂、红斑、疼痛,不良反应发生率及持续时间两组间差异均无统计学意义。1例受试者试验侧和对照侧均出现色素沉着。结论 面部凹陷性瘢痕采用点阵激光联合针刺治疗后加用负压吸引的临床疗效显著,且不会增加患者治疗后不良反应。

Abstract: 【Abstract】 Objective To observe the efficacy and adverse reactions of negative pressure suction combined with fractional laser and pinprick in the treatment of facial atrophic scars. Methods A total of 21 patients with acne atrophic scars were randomly assigned to receive fractional laser and pinprick treatment on one side of the face (control group), while the other side of the face received fractional laser and pinprick treatment followed by negative pressure suction (experimental group). The severity of acne scars was assessed using the échelle d′évaluation Clinique des Cicatrices d′Acné (ECCA) score, Investigator's Visual Assessment (IVA), patient self-assessed overall aesthetic improvement (Global Aesthetic Improvement Scale, GAIS) score, Dermatology Life Quality Index (DLQI), and satisfaction score at 1 and 3 months after the first treatment, and at 1, 3, 6, and 12 months after the second treatment. Adverse reactions and their duration were also recorded. Data were analyzed using SPSS 27.0 software, with between-group comparisons performed using the paired-sample rank sum test. Comparisons at each time point within groups were made using the Friedman two-way repeated measures analysis of variance (ANOVA), with pairwise comparisons adjusted using the Bonferroni correction method. Results Six months after the second treatment, the ECCA score in the experimental group (47.50 [35.00, 50.00]) was lower than that in the control group (50.00 [45.00, 57.75], Z = -2.00, P = 0.045). The IVA score in the experimental group (3.00 [2.00, 3.00]) was higher than that in the control group (2.00 [2.00, 2.50], Z = -2.35, P = 0.019). Over time, both groups showed a decreasing trend in ECCA scores and an increasing trend in IVA scores. At the one-year follow-up, the experimental group had higher satisfaction and GAIS scores than the control group (both P < 0.05). The DLQI score decreased from 6.0 (3.0, 8.5) before treatment to 1.0 (0.0, 1.5) after treatment (Z = -4.02, P < 0.001). The short-term adverse reactions in both groups were primarily scabbing, erythema, and pain. There were no statistically significant differences between the groups in terms of the incidence and duration of adverse reactions. One participant experienced pigmentation on both the experimental and control sides. Conclusion The clinical efficacy of fractional laser combined with pinprick treatment followed by negative pressure suction for facial atrophic scars is significant, and it does not increase the adverse reactions post-treatment.
