传染性软疣82例皮肤镜特征分析

Dermoscopic features of 82 cases of molluscum contagiosum

摘要: 【摘要】 目的 探讨传染性软疣的皮肤镜特征。方法 回顾性分析2019年8月至2020年1月大连市皮肤病医院门诊确诊的82例传染性软疣227 处皮损的皮肤镜特征。结果 227处皮损中,临床检查显示120处(52.86%)可见脐凹和孔,20处(8.81%)可见血管;皮肤镜检中,198处(87.22%)可见脐凹和孔,214处(94.27%)可见血管。皮肤镜下血管分别为皇冠状(132处,58.15%)、点状(34处,15.00%)和混合模式(48处,21.15%)。点状血管模式比例在有炎症或剥脱的病变(86.00%)明显高于无炎症或剥脱的病变(22.03%,χ2 = 81.685,P < 0.001),存在湿疹的病变(50.00%)明显高于没有湿疹的病变(33.85%,χ2 = 17.784,P < 0.001)。白色至黄色无定形结构出现频率由高到低依次为圆形(40.53%)、四叶草形(33.48%)、多叶形(20.26%)、非特异型(5.73%)。结论 传染性软疣在皮肤镜下可见孔、血管结构和黄白色无定形结构,血管的形态模式以皇冠状为主。

Abstract: 【Abstract】 Objective To investigate dermoscopic features of molluscum contagiosum. Methods A total of 82 outpatients with molluscum contagiosum were collected from Dalian Dermatosis Hospital between August 2019 and January 2020, and the dermoscopic features of 227 skin lesions were analyzed retrospectively. Results Of the 227 skin lesions, hilar depression and orifices were observed in 120 (52.86%) by naked eyes and 198 (87.22%) by dermoscopy, and blood vessels were observed in 20 (8.81%) by naked eyes and 214 (94.27%) by dermoscopy. Different vascular patterns were identified by dermoscopy, including crown-shaped vessels (132 lesions, 58.15%), punctate vessels (34 lesions, 15.00%), and a mixed pattern (48 lesions, 21.15%). The proportion of punctate vessels was significantly higher in lesions with inflammation or exfoliation (86.00%) than in those without (22.03%, χ2 = 81.685, P < 0.001), and significantly higher in lesions with eczematous changes (50.00%) than in those without (33.85%, χ2 = 17.784, P < 0.001). The most frequently observed structures were round, white-yellow, amorphous structures (40.53%), followed by four-leaf clover-like pattern (33.48%), polylobular pattern (20.26%) and nonspecific pattern (5.73%). Conclusion Orifices, vascular structures and white-yellow amorphous structures can be observed in molluscum contagiosum by dermoscopy, and the most common vascular pattern is the crown-shaped vessel.
