天疱疮患者外周血单一核细胞糖皮质激素受体mRNA的表达

摘要： 目的 通过检测外周血单一核细胞(PBMC)糖皮质激素受体(GR)mRNA的表达预测天疱疮患者对糖皮质激素(简称激素)治疗的疗效。方法 用逆转录-聚合酶链反应(RT-PCR)方法,对30例天疱疮患者及30例健康对照者PBMCGRα及GRβ的mRNA进行检测。结果 所有天疱疮患者及健康对照者均可检测到GRαmRNA,且天疱疮患者中GRαmRNA的表达水平显著低于正常人对照组(P=0.044),但在激素治疗控制组及未控制组之间,GRαmRNA的表达水平差异无统计学意义。GRβmRNA的检出率:激素治疗未控制组为83%(10/12),激素治疗控制组为17%(3/18),正常人对照组为30%(9/30),GRβmRNA的阳性率在激素治疗控制组与未控制组之间差异有统计学意义(P<0.001)。结论 激素治疗未控制组天疱疮患者PBMC上GRβmRNA表达增加,GRβ在决定天疱疮患者对激素治疗疗效上有一定作用。

Abstract: Objective To clarify whether the mRNA expression level of glucocorticoid receptor (GR) in the peripheral blood mononuclear cells (PBMCs) can predict the clinical response to glucocorticoid (GC) in patients with pemphigus. Methods Reverse-transcription polymerase chain reaction (RT-PCR) was applied to determine the mRNA expression of GRα and GRβ in PBMCs from 30 patients with pemphigus and 30 healthy volunteers. Results GRα mRNA was detected in all patients and all healthy volunteers, and the expression of GRα mRNA in pemphigus patients was significantly lower than that in healthy volunteers (P=0.044), but no significant difference was found between patients insensitive and sensitive to glucocorticoid. In contrast, GRβ mRNA was detected in 10 of 12 patients insensitive to glucocorticoid, 3 of 18 patients sensitive to glucocorticoid, and 9 of 30 healthy Volunteers. The positive rate of GRβ in the insensitive group was significantly higher than that in the sensitive group (P<0.001). Conclusion The results suggest that the expression of GRβ mRNA in PBMCs examined by RT-PCR method is useful in predicting the GC response in pemphigus patients.
