妊娠梅毒的诊断治疗及其临床问题解答

摘要：

妊娠梅毒指妊娠期发现的梅毒螺旋体感染，可在妊娠前及妊娠过程中发生，其严重性不仅危害孕妇的健康，还可以通过垂直传播感染胎儿，造成胎儿先天损害。尽管梅毒有相对特异和敏感的实验室检测方法、有效的治疗手段、诊断治疗成本也不高，但由于梅毒传染性强、发病机制复杂、病程迁延且累及多个器官，目前梅毒的发病率还在逐年增高，几乎可感染所有人群。女性梅毒患者约80%处于育龄阶段，我国妊娠及胎传梅毒发病率近年来增高尤为明显，部分地区孕产妇梅毒感染率高达1.85%［1-2］。流行病学分析显示，我国的妊娠梅毒发病人群以文化程度低、无职业妇女为主。低收入、流动性大、缺乏卫生知识、不重视卫生保健是高发人群的显著特点［1-3］。正因为梅毒在各个阶段及妊娠整个过程中都可能发生垂直传播，妊娠梅毒对母婴造成的严重不良影响已经成为我国重要的公共卫生问题之一。由于妊娠梅毒的传播方式、临床表现、诊断及治疗都有其特殊之处，因此更应该引起高度重视……
