Effect of Q-switched Alexandrite Laser Irradiation on the Dermal Melanocytes in Nevus of Ota

摘要: 目的 研究调Q Alexandrite激光对太田痣黑素细胞的作用机制。方法 在激光照射前及照射后各个阶段,对4例患者分别进行11例次组织病理学观察和14例次透射电镜观察。结果 激光照射瞬间,真皮黑素细胞破坏,黑素小体空泡化,真皮中形成圆形空泡。表皮无明显损伤。照射后3个月至1年间,巨噬细胞吞噬并清除黑素小体和细胞残骸,真皮黑素细胞减少,无纤维组织增生。结论 调Q Alexandrite激光能选择性地破坏真皮黑素细胞,提示能无创伤性治疗太田痣。

Abstract: Objective To investigate the effect of Q-switched Alexandrite laser irradiation on the dermal melanocytes in nevus of Ota. Methods Multiple biopsies were taken in 4 patients with nevus of Ota before and after laser irradiation, altogether 11 biopsies for light microscopic examination and 14 biopsies for transmission electron microscopic examination. Results Immediately after laser irradiation, the dermal melanocytes were destroyed, the melanosomes were disrupted like vacuoles with the formation of intradermal round vacuoles; the epidermis, however, remained intact. Three months to 1 year after irradiation, the disrupted melanosomes and the cell debris were scavenged mainly by macrophages. Dermal melanocytes gradually decreased in number. No fibrosis was found. Conclusion The dermal melanocytes in nevus of Ota can be selectively destroyed by Q-switched Alexandrite laser, without any gross injury.
