抗菌肽LL37对淋球菌的体外抗菌活性研究

Evaluation of in vitro antibacterial activity of antimicrobial peptides LL37 against Neisseria gonorrhoeae

摘要: 【摘要】 目的 探讨抗菌肽LL37对淋球菌的体外抗菌活性。方法 选取中国医学科学院皮肤病医院性病门诊2023年临床分离的75株淋球菌,采用琼脂稀释法检测菌株对抗生素的敏感性。生长抑制实验中,选取3株淋球菌(2株临床分离的多重耐药株和1株标准菌株FA1090)予梯度浓度LL37(7.2、3.6、1.8、0.9、0.45、0.225、0.112 5 μmol/L)或无菌水处理2 h后,制备成菌悬液接种于琼脂板,计数菌落数。选取75株淋球菌及标准菌株FA1090,采用微量肉汤稀释法检测LL37的最低抑菌浓度(MIC)。通过棋盘稀释法检测LL37与头孢曲松对33株多重耐药淋球菌杀伤的联合效应。采用配对样本Wilcoxon符号秩和检验进行统计分析。结果 75株临床分离的淋球菌来自73例男性尿道炎患者及2例女性宫颈炎患者,其中多重耐药菌株33株,非多重耐药菌株42株。生长抑制实验显示,LL37浓度在0.225 ~ 7.2 μmol/L范围内时,3组淋球菌相对活性随LL37浓度升高而降低,至3.6 μmol/L时淋球菌相对活性降到接近于0。LL37对75株淋球菌的MIC为1.8 ~ 7.2 μmol/L,MIC分布的50%位数(MIC50)及MIC90均为3.6 μmol/L。LL37与头孢曲松对33株多重耐药菌株联合药敏实验结果显示,3株(9.1%)为协同作用,30株(90.9%)为无关作用,未见拮抗作用,总体联合效应为无关作用(分数抑菌浓度指数的几何平均数为0.97)。对于28株头孢曲松低敏或耐药株,在联合LL37作用下,头孢曲松MIC下降(Z = -4.21,P < 0.001)。结论 体外实验显示LL37对淋球菌具有抗菌活性,LL37与头孢曲松的联合效应大多数为无关作用,LL37或可用于淋病的治疗。

Abstract: 【Abstract】 Objective To investigate the in vitro antibacterial activity of antimicrobial peptides LL37 against Neisseria gonorrhoeae (N. gonorrhoeae). Methods Seventy-five clinical isolates of N. gonorrhoeae were collected from the STD Clinic, Hospital for Skin Diseases, Chinese Academy of Medical Sciences in 2023. Their antimicrobial susceptibility to antibiotics was determined using the agar dilution method. In the growth inhibition assay, three N. gonorrhoeae strains (two clinically isolated multidrug-resistant strains and one reference strain FA1090) were treated with gradient concentrations of LL37 (7.2, 3.6, 1.8, 0.9, 0.45, 0.225, and 0.112 5 μmol/L) or sterile water for 2 hours. Bacterial suspensions were then prepared and inoculated onto agar plates, and colony-forming units were counted. The minimum inhibitory concentrations (MICs) of LL37 against the 75 clinical isolates and the reference strain FA1090 were determined using the broth microdilution method. The combined bactericidal effect of LL37 and ceftriaxone against 33 multidrug-resistant N. gonorrhoeae strains was evaluated using the checkerboard dilution method. Statistical analysis for paired samples was performed using the Wilcoxon signed-rank test. Results Seventy-five clinical isolates of N. gonorrhoeae were obtained from 73 male patients with urethritis and 2 female patients with cervicitis, including 33 multidrug-resistant strains and 42 non-multidrug-resistant strains. The growth inhibition assay showed that the survival rates of the three N. gonorrhoeae strains decreased with an increase in LL37 concentrations (range, 0.025 - 7.2 μmol/L), and approached 0 at LL37 concentrations ≥ 3.6 μmol/L. The MICs of LL37 against the 75 N. gonorrhoeae isolates ranged from 1.8 to 7.2 μmol/L, and both the MIC?? and MIC?? were 3.6 μmol/L. The combined antimicrobial susceptibility test of LL37 and ceftriaxone against 33 multidrug-resistant strains showed synergistic effects in 3 strains (9.1%) and indifferent effects in 30 strains (90.9%), with no antagonistic effect observed. The overall combined effect of LL37 and ceftriaxone was indifferent (geometric mean of the fractional inhibitory concentration index = 0.97). Among the 28 strains with reduced susceptibility or resistance to ceftriaxone, the MIC of ceftriaxone decreased after combination with LL37 (Z = ?4.21, P < 0.001). Conclusions In vitro experiments showed that LL37 had antibacterial activity against N. gonorrhoeae. The combined effect of LL37 and ceftriaxone was indifferent for most strains. LL37 may have potential for the treatment of gonorrhea.
