钙泊三醇倍他米松软膏治疗寻常性银屑病临床疗效观察

Efficacy of calcipotriol betamethasone ointment for the treatment of psoriasis vulgaris: a clinical observation

摘要 目的 评价钙泊三醇倍他米松软膏治疗寻常型银屑病的疗效及安全性。方法 将90例寻常型静止期银屑病患者随机分为3组，治疗组予钙泊三醇倍他米松软膏，对照1组予卡泊三醇软膏，对照2组予糠酸莫米松乳膏，均外用1次/d，疗程4周。治疗1周后，2周后和4周后分别对皮损进行PASI评分和不良反应记录。结果 4周治疗完成后，治疗组总有效率为73.33%，显著高于两对照组（50%，46.67%；P均＜0.01）；4周后治疗组PASI评分较基线下降百分比为0.77±0.16，显著高于两对照组（0.57±0.15，0.54±0.16；P均＜0.01）；治疗4周后PASI评分，治疗组均低于两对照组（P均＜0.05）。治疗组和对照1组各有1例患者在治疗结束时发现血钙轻度升高，无系统症状，停药1周后复查血钙恢复正常，5例出现局部刺激症状但均能耐受。结论 钙泊三醇倍他米松软膏外用治疗银屑病具有起效快、疗效好、相对安全等特点，使用方便，患者依从性好

Abstract:

【Abstract】Objective To assess the clinical efficacy and safety of calcipotriol betamethasone ointment (Daivobet?) in Chinese patients with psoriasis vulgaris. Methods 90 patients were randomly divided into three groups to receive calcipotriol betamethasone ointment, calcipotriol ointment or mometasone furoate ointment once daily for 4 weeks. The safety and efficacy of the three groups were evaluated on week1, 2 and 4 after the first dosage. Results After 4 weeks of treatment, effective rate of calcipotriol betamethasone-treated patients was much higher than calcipotriol-treated patients and mometasone furoate-treated patients (73.33% vs 50% vs 46.67%;P＜0.01), psoriasis area and severity index(PASI) of calcipotriol betamethasone-treated patients was much lower than the other two groups (P＜0.05), and calcipotriol betamethasone-treated patients experienced a greater decrease in PASI compared with the other two groups (0.77±0.16 vs 0.57±0.15 vs 0.54±0.16;P＜0.01). One calcipotriol betamethasone-treated patient and one calcipotriol-treated patient received increased serum calcium without syndromes after 4 weeks treatment, which decreased to normal value after drug withdrawal for 1 week. Five calcipotriol betamethasone-treated patients and five calcipotriol-treated patients receiced tolerated local stimulation on test positions during therapy. Conclusions Daily calcioptriol betamethasone ointment is an effective, safe, convenient , well-tolerated treatment with a rapid onset of action for stable psoriasis vulgaris in Chinese patients.
