山东汉族关节病性银屑病与HLA-A、B等位基因相关性研究

Relationship between human leukocyte antigen-A, -B alleles and psoriatic arthritis in Chinese Han population from Shandong Province

摘要: 目的 探讨HLA-A、B等位基因与山东汉族关节病性银屑病及其临床特征的相关性。方法 PCR-寡核苷酸探针杂交分型法(PCR-SSOP)对42例山东汉族关节病性银屑病患者(患者组)与90例健康献血者(对照组)进行HLA-A、B等位基因分型。结果 两组比较,关节病性银屑病患者组B*27等位基因频率(患者组为11%,对照组为1.8%,Pc < 0.01)、B*57等位基因(分别为4.8%和0.4%,Pc < 0.05)及B*13/B*27杂合体(分别为8.5%和0.5%,Pc < 0.01)的出现频率明显高于对照组,A*33等位基因频率(分别为0.6%和6.7%,Pc < 0.01)明显低于对照组。B*27等位基因频率在关节病性银屑病脊柱受累组为80%,无脊柱受累组为 22%(两组比较,P < 0.01);外周附着点受累组为63%,无外周附着点受累组为23%(P < 0.05)。7例B*13/27杂合体阳性者中5例(71%)有脊柱炎合并外周关节炎,而36例B*13/27杂合体阴性者中有8例(22%),两组比较,χ2 = 6.73,P < 0.05。结论 HLA-B*27与山东汉族关节病性银屑病相关。

Abstract: Objective To investigate the relationship of human leukocyte antigen (HLA)-A, B alleles to psoriatic arthritis (PsA) and its clinical subtypes in Chinese Han population from Shandong Province. Methods Polymerase chain reaction-sequence specific oligonucleotide probe (PCR-SSOP) method was used to determine HLA-A, B alleles in 42 consecutive PsA patients and 90 healthy human controls of Han Nationality from Shandong Province. Results A significant increase was observed in the frequencies of HLA-B*27 (11% vs 1.8%, P < 0.01), B*57(4.8% vs 0.4%, P < 0.01) alleles and B*13/B*27 heterozygote (8.5% vs 0.5%, P < 0.01) in PsA patients than those in the healthy controls, together with a decreased frequency of A*33 allele(0.6% vs 6.7%, P < 0.001). In PsA patients, there was a significant difference in the frequency of B*27 allele between patients with spine involvement and those without (80% vs 22%, P < 0.01), as well as between those with peripheral enthesis involvement and those without (63% vs 23%, P < 0.05). Spondylitis incorporating peripheral arthritis was noted in 5 out of 7 patients positive for HLA B*13/B*27 heterozygote, compared to 8 out of 36 negative for HLA B*13/B*27 heterozygote (χ2 = 6.73, P < 0.05). Conclusion The HLA-B*27 allele is correlated with PsA in Chinese Han population from Shandong Province.
