梅毒血清学检测质量亟待提高

Performance of syphilis serology is urgently needed to be improved

摘要:

目的:了解本市各级医疗机构实验室开展梅毒血清学检测质量。方法:将一组样品发送到各实验室进行检测,比较每一实验室的结果与同组的其它实验室的结果/靶值,统计和分析反馈结果。结果:212家实验室结果回报率达到99%。在严格的试验要求和判断标准的条件下,虽然确诊试验合格率(90.6%)高于筛查试验(72.8%),但所有检测全部合格的医院不到三成。不同靶值样品的筛查试验,高滴度的合格率好于低滴度。在部分现场抽查的实验室中,筛查试验时使用水平摇床的仅占28%。结论:弱阳性或者临界样品的检测结果,是影响梅毒血清学室间质量评价质量的主要因素之一。不同级别的医疗机构,存在着检测质量的总体差异。需要我们探索提高检测质量的方法。

Abstract:

Objective: To investigate the quality status of serologic detection for Treponema pallidum (TP) in clinical laboratories of Shanghai. Methods A panel consisting of fives serologic samples including negative, lower-titer positive and higher-titer positive samples were distributed for each laboratory. The participants were required to detect these samples used them examine system and return the data. All date were entered the computer and statistical analyzed with SPSS 12.1, and to compared the results of detection with the target value of the samples. Results The rate of feedback from total 212 participant laboratories was up to 99%. The qualification ratio was 90.6% in treponema test, and 72.8% in nontreponema test, respectively. The eligible laboratory of both test for all these samples were below 30%. Among screening tests, the qualification ratios in higher titer were above those in lower titer. The use ratio of mechanical horizontal rotators in screening tests was only 28% on spot checks. conclusion There are some problem in detecting weakly positive sample of screening tests in clinical laboratories of Shanghai. The results of nontreponema testing in weakly positive sample were the key factor to impact the quality status, and had significant difference in different grades hospitals. This means that it is necessary to improve the detection quality of the laboratories.
