上海地区女性外阴尖锐湿疣患者宫颈HPV感染及分型研究

Relationship between Vulvar Condylomata Acuminata Lesion and Cervical HPV Infection in Patients of Shanghai Area

[摘要] 目的 探讨上海地区女性外阴尖锐湿疣患者的宫颈HPV感染情况。方法 采用美国FDA批准的捕获杂交试验和基因芯片技术对194例女性外阴尖锐湿疣患者皮损和宫颈脱落细胞进行HPV 基因型别研究。结果 194例患者的女性外阴皮损HPV检出率为100%,其中低危型HPV为74.2%(144/194),高危型HPV为25.8%(50/194);低危型以单一型别感染为主, 占94.4%(136/144),均为HPV6或HPV11。 高危型以混合型别感染为主,86%(43/50)患者为低危与高危型HPV混合感染。194例患者的宫颈脱落细胞中,除了14.4%(28/194)的患者未合并HPV感染外,有85.6%(166/194)患者合并宫颈HPV感染,其中低危型感染者61.4%(119/194),高危型感染率高达23.7%(46/194)。外阴和宫颈皮损HPV型别的一致性达到95.8%(159/166),低危型HPV感染率从高到低依次为HPV11>HPV6>HPV53,高危型依次为HPV16>HPV18>HPV52>HPV31 >HPV45、HPV58。结论 上海地区女性外阴高危型HPV感染率较高,且近1/4患者合并宫颈高危型HPV感染;提示皮肤科医生应关注和加强女性尖锐湿疣患者宫颈高危型HPV感染情况与严密随访,这将对降低女性宫颈癌发生率有着极其重要的意义。

Abstract:

[Abstract] Objective: To study the relationship between vulvar condylomata acuminata lesion and cervical HPV infection in female patients of Shanghai area. Methods: HPV DNA typing of 194 specimens from acuminata lesions and cervical cell collections was performed using hybridization and gene chip techniques. Results: HPV were detected in all 194 patients (100%). Among them, LR-HPV and HR-HPV infection was 74.2% (144/194) and 25.8% (50/194), respectively. LR-HPV infection was mainly characterized as a single HPV type (94.4%, 136/144) – either HPV6 or HPV11. In contrast, 86% (43/50) of HR-HPV infection were mixed with LR-HPV. Cervical cell examination showed that 85.6% (166/194) of patients were accompanied with HPV infection (LR = 61.4%, HR = 23.7%). In term of HPV type, the correlation between acuminata lesion and the cervix was 95.8%. The order of LR-HPV was HPV11>HPV6>HPV53 and that of HR-HPV was HPV16>HPV18>HPV52>HPV31 >HPV45 and HPV58. Conclusions: The infection rate of HR-HPV was high among female acuminata patients in Shanghai area and 1/4 of them were accompanied with HR-HPV infection of cervix. Therefore, dermatologists should pay attention to such infection in order to reduce the cervical cancer associated with HR-HPV infection.
