过敏原免疫治疗对慢性荨麻疹患者单一核细胞IL-2、4受体表达的影响

Effect of allergen immunotherapy on the expression of interleukin-2 and -4 receptors by mononuclear cytes in patients with chronic urticaria

摘要:

目的 通过对比过敏原免疫治疗(AIT)前后慢性特发性荨麻疹(CIU)患者体内IL-2、4受体的变化,初步探索AIT对CIU患者免疫的影响,CIU的发病机制及AIT治疗CIU的机制。方法 临床选择35例CIU患者,实验组20例,对照组15例。实验组给予AIT干预。对照组仅抗组胺药对症治疗。实验前后分别采集静脉全血,提取总的RNA,逆转录-PCR扩增IL-2R、4R及内参照β肌动蛋白mRNA的片断。目的条带的峰面积除以内参照β肌动蛋白的条带峰面积,作为目的蛋白mRNA表达的半定量值,结果用 表示,统计分析采用配对资料t检验。结果 实验组实验前后IL-2R mRNA的表达量分别为0.356±0.111,0.370±0.092;IL-4R mRNA的表达量分别为0.483±0.087,0.474±0.076。对照组实验前后IL-2R mRNA的表达量分别为0.330±0.111,0.335±0.093;IL-4R mRNA的表达量分别为0.511±0.080,0.527±0.086。通过配对t检验分析,比较实验前后每组IL-2R、4R mRNA的表达量,P值均大于0.05,没有统计学意义。实验组中,AIT疗效达到良好及其以上的共13例,以13例作为一组分析,实验前后其IL-2R mRNA表达量分别为0.338±0.122,0.359±0.104;IL-4R mRNA表达量分别为0.485±0.092,0.471±0.084。通过配对t检验分析,P值均大于0.05,没有统计学意义。结论 AIT疗效良好的实验组在实验前后IL-2R,IL-4R mRNA的表达没有统计学差异,说明CIU的发病并不是单纯的Th2表达模式,同时说明免疫偏移不是AIT治疗CIU的主要机制。

Abstract:

Objective  To compare the mRNA expression of interleukin 2 (IL-2) and 4 (IL-4) receptors in the patients with chronic idiopathic urticaria (CIU) before and after allergen immunotheropy (AIT). Methods 35 patients with CIU were enrolled in this study, divded into group 1 with AIT and group 2 without AIT. The peripheral blood was extracted before and after test. Reverse transcription-polymerase chain reaction (RT-PCR) technique was applied to semi-quantitatively analyze the mRNA expression of IL-2 and IL-4 receptors. Results Before and after test, the mRNA expressions of IL-2 receptor were 0.356±0.111,0.370±0.092 respectively in group 1, and 0.330±0.111,0.335±0.093 in group 2. The mRNA expression of IL-4 receptor were 0.483±0.087,0.474±0.076 in group 1, and 0.511±0.080,0.527±0.086 in group 2. It showed no significant difference before and after test for IL-2 or IL-4 receptor in each group. Among the 13 cases of group 1 with good responses to AIT, the expressions of IL-2 receptor were 0.338±0.122,0.359±0.104 before and after test. And the expressions of IL-4 receptor were 0.485±0.092,0.471±0.084. There was also no significant difference. Conclusion Th2 model is not the mechanism of CIU. Re-balance between Th1 and Th2 is not the main mechanism of AIT to treat CIU.
