皮下脂膜炎样T细胞淋巴瘤的临床病理和免疫组化研究

Subcutaneous Panniculitis like T-cell Lymphoma:A Clinicopathological and Immunohistochemical Study of 12 Cases

摘要： 目的 认识皮下脂膜炎样T细胞淋巴瘤的临床和组织学特征.方法 临床资料分析、组织形态学观察、免疫组化标记.结果 男女各6例,中位数年龄26岁,主要表现为下肢、躯干无症状性结节或肿块,3例伴发热、消瘦等症状.做血常规检查的6例中3例血象降低.获随访的7例中3例死亡.组织学见肿瘤在皮下脂肪内呈脂膜炎样浸润;瘤细胞大小不一,形态不规则,核分裂较少;瘤内可见上皮样肉芽肿、多核巨细胞、豆袋细胞、小片坏死和瘤细胞浸润小血管壁.12例肿瘤均表达T细胞标记.结论 皮下脂膜炎样T细胞淋巴瘤有独特的临床病理特点,伴噬血细胞增生症、瘤细胞大者预后较差.

Abstract: Objective To study the clinical and histopathological features of subcutaneous panniculitis like T-cell lymphoma (SPTCL).Methods Twelve cases of SPTCL were investigated by clinicopathological and immunohistochemical methods.Results The ratio of male-to-female in this group was 1:1,and the median age was 26 years old The main clinical manifestations of SPTCL were asymptomatic plaques,nodules and masses Fever and weight loss were seen in three cases,and low level cell counts in routine blood test in 3 patients In seven followed up patients three died Histologically,patterns of lymphoid cell infiltration were lobular or diffuse panniculitis,or both The infiltrated lymphoid cells were of different size,irregular in shape,with few mitotic figures In tumor tissue,epithelioid granulomas,multinuclear giant cells(4/12),beanbag cells (7/12),small patches of necrosis(7/12),and tumor cell infiltrating small vessel walls (6/12)were observed CD45RO positive T-cells were shown in all 12 cases by immunohistochemical stain Conclusion s SPTCL has a specific clinicopathological picture,and the prognosis is poor in those with hemophagocytic syndrome or large tumor cells.
