系统性红斑狼疮患者抗中性粒细胞胞浆抗体和抗心磷脂抗体的检测

Detection of Antibodies Against Neutrophil Cytoplasmic Antigens(ANCA) and Anticardiolipin Antibodies (ACA) in Patients with SLE

摘要: 目的 探讨抗中性粒细胞胞浆抗体 (ANCA)及抗心磷脂抗体(ACA)与SLE血管损害的关系。方法 99例活动期SLE患者,以间接免疫荧光技术检测血清中ANCA,以酶联免疫吸附方法检测血清中ACA。结果 具有皮肤血管炎表现的SLE组ANCA阳性率为30.8% (39例中12例阳性),无血管炎表现的SLE患者ANCA阳性率为8.3% (60例中5例阳性),两组间有非常显著性差异 (P<0.01)。 ACA检测结果显示两组间无显著性差异 (P>0.05)。同时ANCA阳性组与阴性组比较,ACA阳性组与阴性组比较,在肾脏损害、关节症状、雷诺现象及低补体、抗dsDNA抗体或抗核抗体阳性等方面无显著性差异 (P>0.05)。结论 检测ANCA对于SLE合并皮肤血管炎具有一定意义,其对于SLE合并内部器官血管炎的意义尚需进一步研究。

Abstract: Objective To study the relationship between antineutrophil cytoplasmic autoantibodies (ANCA) and anticardiolipin antibodies(ACA) and vascular injury in patients with SLE.Methods We examined ANCA/ACA in the sera from 99 patients with active SLE by indirect immunoflourescence technique and enzyme linked immunosorbent assay(ELISA) reapectively.Results The positive rate of ANCA in SLE patients with cutaneous vasculitic manifestation was 30.8% (12/39),and the positive rate in SLE patients without cutaneous vasculitis was 8.3% (5/60),the difference between the two groups was very significant (P<0.01). There was no significant difference in the positive rate of ACA between these two groups(P>0.05).These two kinds of antibodies were neither significantly correlated with renal involve-ment,arthritis,Raynaud’s phenomenon,nor with hypocomplementemia,antinuclear antibodies and anti-dsDNA antibodies.Conclusion ANCA detection may be useful to identify cutaneous vasculitis in SLE, but the value in the diagnosis of vasculitis of internal organs SLE needs to be further studied.
