Three cases of AIDS-associated Fourniers gangrene

Abstract:

【Abstract】OBJECTIVE:Fournier's gangrene(FG) is a rare but life-threatening necrotizing fascitis occurring around the genitals, caused by a polymicrobial infection. The aim of study was to initially explore the incidence of FG in HIV-infected patients and to share our experiences of treating AIDS patients with FG and their outcomes. METHODS:A descriptive analysis was carried out on three FG patients among 377 HIV-infected adult male patients who seeked medical care or antiretroviral therapy (ART) in HIV clinic at Jinja hospital of Uganda from Aug.2005 to Jul.2007 and the clinical features of 3 AIDS patient with FG which diagnosis established on the basis of history and clinical examination were summarized. RESULTS: The incidenceof FG among HIV-infected adult male patients in Jinja of Uganda was 0.80%. Three patients were admitted which ages were betwen 23 to 32 years.Two patients were taking ART. One patient had scrotum slight trauma and 2 had scortum scratching before presenting with infection.The histories were 9,6,4 days respectively before admission while gangrenous patches widely. CD4 counts ranged from 86 cells/mm3 to 243cells/mm3. All patients received broad spectrum antibiotics, of 2 underwent regular debridement. 2 patients died and one heal.CONCLUSION: Fournier's gangrene associated with AIDS is a rare, rapidly progressive, but life threatening condition with high associated mortality. Immunosuppression was related to death. Early surgical debridement and antibiotics were the most important therapeutic measures. 【Key words】Fournier's gangrene , AIDS
