采用AIS、SAS、HAMD对469例痤疮患者进行心理评估

Psychological evaluation of 469 patients with acne by using Athens insomnia scale, self-rating anxiety scale and Hamilton depression scale

摘要:

目的 探讨痤疮对患者心理的影响。方法 对469例痤疮患者和175例健康对照进行问卷调查,使用睡眠自测量表(AIS)、焦虑自评量表(SAS)、Hamilton抑郁他评量表(HAMD)分别评价患者与健康对照失眠、焦虑、抑郁状况,并通过秩和检验进行比较。结果 痤疮患者组AIS(Z = 4.014,P < 0.01)、SAS(Z = 3.426,P < 0.01)、HAMD(Z = 8.664,P < 0.01)评分均高于健康对照组,其中失眠、焦虑、抑郁在痤疮患者中的发生率分别为34.9%、0.7%、2.9%。痤疮组296例女性患者AIS评分比173例男性高(Z = 2.009,P < 0.05),而SAS、HAMD评分差异无统计学意义(P值均 > 0.05);不同发病年龄段间(< 15岁、15 ~ 25岁、> 25岁)患者失眠、焦虑、抑郁发生率差异无统计学意义(P > 0.05);重度痤疮患者HAMD评分较轻度痤疮患者高(Z = 1.826,P < 0.01),而AIS、SAS评分差异无统计学意义(P > 0.05)。结论 痤疮影响患者的睡眠、焦虑、抑郁情绪,从而影响患者的心理健康,尤其是重度痤疮患者与女性痤疮患者的心理更容易受疾病影响。

Abstract:

Objective To evaluate the psychological impact of acne on patients. Methods A questionnaire survey was conducted among 469 patients with acne and 175 healthy controls. Athens insomnia scale(AIS), self-rating anxiety scale(SAS) and Hamilton depression scale(HAMD) were used to estimate the prevalence of insomnia, anxiety and depression respectively in these subjects. Rank sum test was performed to compare the differences in psychological status between the patients and controls. Results Increased AIS, SAS and HAMD scores were observed in the patients with acne compared with the healthy controls(Z = 4.014, 3.426, 8.664, respectively, all P < 0.01). The incidence of insomnia, anxiety and depression was 34.9%, 0.7% and 2.9% respectively in the patients with acne, with no significant differences between patients in different age groups (< 15 years, 15-25 years, > 25 years, all P > 0.05). A significant increase was observed in AIS score in 296 female patients compared with 173 male patients(Z = 2.009, P < 0.05), and in HAMD score in patients with severe acne compared with those with mild acne(Z = 3.739, P < 0.01). There was no statistical difference in SAS or HAMD score between the male and female patients, or in AIS or SAS score between patients with severe and mild acne. Conclusions Acne could affect the sleep, anxiety and depression emotions in patients, and exert a negative psychological impact on patients, which seems more apparent in patients with severe acne and female patients.
