遗传过敏性皮炎与HLA抗原相关的研究

摘要： 本文对186例汉族遗传过敏性皮炎(AD)患者[纯AD(PAD)42例、AD伴呼吸道过敏(ADR)62例,AD伴鱼鳞病(ADI)33例,AD伴呼吸道过敏及鱼鳞病(ADRI)49例]进行了HLA-A,B抗原分型,其中63例(PAD30例,ADR33例)同时作了DR抗原分型.并分别与220例正常人对照(DR抗原对照为105例)相比较,观察到全部患者(ADt)和ADI组Az抗原频率显著增高,校正后仍有显著性.63例AD患者中DR5抗原频率增高,校正P值处于临界(Pc≈0.005.RR=2.78).发现了10个相对易感AD的单倍型,A30-B13为相对抵抗各型AD的单倍型.进一步分析发现:1.女性患者与A2抗原相关(RR=2.42);2.家族遗传过敏史与B7抗原相关(RR=3.70.食物过敏与B48抗原相关(RR=11.92),四型AD为多抗原弱相关,相关抗原及单倍型不同,提示AD可能是一组异质性疾病.

Abstract: The HLA-A,B antigen typing was invcstigated in 186 patients of Han nationality with atapic dermatitis (AD) (among them 42 PAD,62 ADR,33 ADI and 49 ADRI),compared to 22D healthy controls.Fwther more the DR antigen was studied in 63 patients (between them 30 PAD and 33 ADR),together with 105 controls.Increased frequency of A2 antigen wag shown statistically in all patient group (ADt) as well as in ADI subset,even after corrested for the number of A,B antigens tested (Pc=0.0209,RR=1.89 and Pc=0.0143,RR=3.64,respectively).The increased frequency of DR5 antigen in 63 patients,corrested P value was on the critical value (Pc≈0.05,RR=2.78).Ten haplotypes relatively susceptible to A were found,and A30-B13 was relatively resistant to ADt and four subsets of AD.Further analysis showed: 1.A2 antigen correlated strongly with floe female patients (RR=2.42); 2.B7 antigen correlated with family history of atopy (RR=3.70);3.B48 antigen correlated with fond allergy (RR=11.92).The associations among PAD,ADR,ADI or ADRI and HLA were multiantigenic and weak,and their relative antigens and haplotypes were different.The authors suggest that AD appears to be a disease of heterogeneous nature.
