枝孢样枝孢所致皮肤暗色丝孢霉病一例

A case of subcutaneous phaeohyphomycosis due to Cladosporium cladosporioides

摘要: 目的 报道国内首见枝孢样枝孢所致的皮肤暗色丝孢霉病1例.方法 患者女,49岁,右臀部肿块近2年,瘢痕纤维化的同时有新的肿块形成,并形成囊肿坏死、窦道形成,皮肤表面破损,大量渗液,触痛明显,皮损边界基本清晰.从患者皮损取材、涂片及培养,并经致病性研究重现组织病理学特征.结果 培养菌落呈绒状,橄榄绿到橄榄棕色,反面为橄榄黑色.显微镜下,分生孢子梗末端和外侧可产生枝状分生孢子链,分生孢子椭圆形或柠檬形,壁光滑.根据直接镜检和培养的菌落形态及显微镜下分生孢子的大小、形态、排列及培养温度试验等特征,鉴定为枝孢样枝孢.结论 该病例为国内首报的由枝孢样枝孢引起的暗色丝孢霉病.

Abstract: Objective To report the first case of subcutaneous phaeohyphomycosis caused by Cladospotium cladosporioides in China.Methods The patient was a 49-year-old female,presented with a tumorous mass on the right hip for about two years.New lesions and fibrous scar developed simultaneously.Physical examination revealed relatively well-demarcated lesions of various morphology,including necrotic cysts,sinuses and breakage on the skin surface with a large amount of effusion;all were markedly tender.Smear and fungal culture were carried out on specimens collected from the patient's lesions.Histopathological features were reproduced through an animal pathogenicity study in a murine model.Results Fungal culture of the isolate at 25℃ resulted in olivaceous green to olivaceous brown colonies with velvety surface;the color of the undersurface was olivaceous black.Microscopic examination showed branched chains of conidium developing at the terminal and lateral sides of conidiophores,which were oval or citriform with smooth walls.The isolate was identified as Cladosporium cladosporioides based on its biological characteristics.Conclusion This is the first case report of subcutaneous phaeohyphomycosis caused by Cladosporium cladosporioides in China.
