2008—2023年河南省单中心孢子丝菌病流行病学及临床特征分析

Epidemiological and clinical analysis of sporotrichosis in Henan Province from 2008 to 2023

摘要: 【摘要】 目的 分析河南省单中心近15年孢子丝菌病流行病学及临床特征。方法 回顾性分析2008—2023年河南省人民医院皮肤科确诊为孢子丝菌病患者的资料,并对保存菌株做分子生物学鉴定以分析致病菌种。结果 共收集孢子丝菌病病例83例,其中男32例,女51例,年龄[M(Q1,Q3)]59(50,69)岁,范围2 ~ 88岁,患者主要在40岁以上(70例,84.3%);病程1 ~ 24个月,平均6.9个月;居住于农村地区者65例(78.3%);71例成人患者中,职业以务农者(54例,76.1%)为多;有明确外伤史者28例(33.7%)。发病季节主要为秋季(37例,44.6%)、春季(20例,24.1%)。感染地区分布中,驻马店地区25例,周口地区16例,郑州地区9例,平顶山地区8例。皮损位于上肢55例,面部22例,下肢4例,颈部2例。皮损表现多样,包括红肿、丘疹、结节、斑块、溃疡等。疾病临床分型以固定型(52例,62.7%)更常见。成人患者与儿童患者发病部位及临床分型差异有统计学意义(均P < 0.001)。治疗采用口服特比萘芬、伊曲康唑或10%碘化钾;58例(69.9%)患者完成随访,均治愈,疗程(3.4 ± 0.7)个月。皮损培养菌株共活化21株,rDNA内转录间隔区测序、钙调蛋白基因测序显示致病菌种均为球形孢子丝菌。结论 本研究显示,河南省孢子丝菌病多见于40岁以上人群,从事务农劳动者易感染,感染部位以暴露部位为主,尤其是上肢,其次为面部;患者临床常表现为同时存在多种皮损,以固定型多见;感染区域特征明显,主要位于河南省南部地区。致病菌种为球形孢子丝菌。

Abstract: 【Abstract】 Objective To analyze the epidemiological and clinical characteristics of sporotrichosis in Henan Province over the past 15 years. Methods Clinical data were collected from patients diagnosed with sporotrichosis in the Department of Dermatology, Henan Provincial People's Hospital from 2008 to 2023, and were retrospectively analyzed. Preserved clinical isolates were identified by molecular biological methods. Results A total of 83 patients with sporotrichosis were collected, including 32 males and 51 females. Their age (M[Q1, Q3]) was 59 (50, 69) years, with the range being 2 - 88 years; the majority of patients were aged > 40 years (70 cases, 84.3%); the disease duration ranged from 1 to 24 months (mean: 6.9 months); 65 patients (78.3%) resided in rural areas; among the 71 adult patients, the majority were farmers (54 cases, 76.1%); 28 patients (33.7%) had a definite history of trauma. The main seasons of disease onset were autumn (37 cases, 44.6%) and spring (20 cases, 24.1%). In terms of geographic distribution, 25 patients were from Zhumadian, 16 from Zhoukou, 9 from Zhengzhou, and 8 from Pingdingshan. Skin lesions were located on the upper limbs in 55 cases, on the face in 22 cases, on the lower limbs in 4 cases, as well as on the neck in 2 cases. In addition, skin lesions presented in various forms, including erythema, swelling, papules, nodules, plaques, and ulcers. The fixed type was the most common clinical type (52 cases, 62.7%). There were significant differences in lesional sites and clinical types between adult and pediatric patients (both P < 0.001). Treatment involved oral administration of terbinafine, itraconazole, or 10% potassium iodide; 58 patients (69.9%) completed follow?up and were all cured, with a treatment duration of 3.4 ± 0.7 months. A total of 21 clinical isolates were reactivated, and rDNA internal transcribed spacer region sequencing and calmodulin gene sequencing showed that the pathogenic species were all Sporothrix globosa. Conclusions Sporotrichosis predominantly affected individuals aged > 40 years in Henan Province, especially agricultural workers. The lesions mainly occurred on exposed areas, particularly the upper limbs, followed by the face. Patients often presented with multiple types of skin lesions simultaneously, with the fixed type being the most common. Distinct regional characteristics were observed, with cases concentrated in the southern part of Henan Province. The causative species was identified as Sporothrix globosa.
