微创旋切术治疗腋臭患者3 000例术后并发症回顾分析

Postoperative complications in 3 000 patients with axillary osmidrosis after minimally invasive rotary cutting surgery: a retrospective study

摘要： 【摘要】 目的 探讨微创旋切术治疗腋臭患者术后并发症及预后情况。方法 回顾性收集南华大学衡阳医学院附属第二医院烧伤整形科2012年1月至2022年1月行微创旋切手术治疗的腋臭患者资料，分析临床疗效及术后相关并发症的发生情况，记录患者术后并发症发生的类型及针对性的预防和治疗方法以及随访结局。结果 共纳入3 000例腋臭患者（6 000侧腋窝），其中男性1 180例，女性1 820例，年龄18～40岁，平均年龄25岁。手术时长30～60min，平均48 min。随访1年，其中5 640侧（94%）腋臭治愈，360侧（6%）腋臭显效，无效0侧。并发症包括局部血肿360侧、皮肤折叠37侧、表皮磨损370侧、皮肤坏死10侧、增生性瘢痕5侧，针对上述并发症分别予以早期清除血肿，机械拉伸折叠皮肤，磨损、坏死皮肤加强换药，点阵激光抗瘢痕治疗等方法，均得到缓解。结论 微创旋切刀治疗腋臭术后并发症总体发生率约为13%，加强对术中细节的把控及术后护理的宣教可以有效避免相关并发症的发生。

Abstract: 【Abstract】 Objective To investigate postoperative complications in and prognosis of patients with axillary osmidrosis after minimally invasive rotary cutting surgery. Methods Clinical data were retrospectively collected from patients with axillary osmidrosis who underwent minimally invasive rotary cutting surgery in the Department of Burn and Plastic Surgery, the Second Affiliated Hospital of Hengyang Medical College, University of South China from January 2012 to January 2022. Clinical efficacy was analyzed, and postoperative complications were investigated. Types of postoperative complications, as well as targeted prevention and treatment methods and follow-up outcomes, were recorded. Results A total of 3 000 patients with axillary osmidrosis （6 000 axillae） were included, comprising 1 180 males and 1 820 females. They were aged 18 - 40 years, with an average age of 25 years. The surgical duration was 30 - 60 minutes, with an average of 48 minutes. During 1 year of follow-up, 5 640 affected axillae （94%） were cured, 360 axillae （6%） were markedly improved, and no axilla showed no response. Postoperative complications included local hematoma in 360 axillae, skin folds in 37 axillae, epidermal abrasion in 370 axillae, skin necrosis in 10 axillae, and hypertrophic scars in 5 axillae. Early removal of hematoma, mechanical stretching of folded skin, enhanced dressing changes for abraded and necrotic skin, and anti-scar treatment with fractional laser, etc., were performed for the aforementioned complications, and all of the complications were relieved. Conclusions The overall incidence rate of postoperative complications was about 13% in patients with axillary osmidrosis after minimally invasive rotary cutting surgery. Strengthening the control of intraoperative details and postoperative care education could effectively avoid the occurrence of related complications.
