尖锐湿疣患者外周血中HPV DNA的检测

Detection of HPV DNA in Peripheral Blood of Patients with Condyloma Acuminata

摘要: 目的 检测尖锐湿疣(CA)患者外周血中的人乳头瘤病毒(HPV)DNA。方法 采用聚合酶链反应(PCR)检测30例CA患者皮损和外周血中的HPV DNA,20例正常人外周血作对照。采用限制性内切酶RsaⅠ、PstⅠ对HPV作分型鉴定。选取1例皮损与外周血均为阳性的PCR产物进行测序。结果 30例CA患者皮损均检出HPV DNA,血浆中未检测到HPV DNA,有8例外周血单一核细胞(PBMC)中检测到HPV DNA(阳性率为26.7%),20例正常人对照为阴性,两组阳性率经χ2校正检验χ2=4.52,P<0.05,差异有显著性。酶切和测序结果显示PBMC中的HPV型别与其皮损处的型别有一致性。结论 CA患者在HPV感染的病程中可能存在病毒血症,这可能是CA易于复发的原因之一。

Abstract: Objective To detect human papillomavirus(HPV)DNA in peripheral blood of patients with condyloma acuminata(CA).Methods Polymerase chain reaction(PCR)was used to amplify the HPV DNA in skin lesions and peripheral blood of 30 patients with CA,and 20 normal blood donors as controls.Re-striction endonucleases Rsa I and Pst I were used for HPV DNA typing.The PCR products from 1 patient who was HPV positive for both skin lesions and peripheral blood,were cloned and sequenced.Results HPV DNA was identified in all CA skin lesions.HPV DNA was identified in peripheral blood mononuclear cells(PBM-Cs)from 8 of 30(26.7%)patients and in none of 20 control subjects(P<0.05).HPV DNA was not found in the plasma.In all 8 patients who had positive results for both skin lesions and PBMCs,the results of DNA sequence and endonuclease cleavage showed that the types of HPV found in the skin lesions were in accordance with those found in the PBMCs.Conclusion These findings suggest that viremia of HPV may occur in CA,and HPV DNA in PBMCs may serve as a source of virus reinfection.
