系统性红斑狼疮患者外周血淋巴细胞早期凋亡的初步研究

Preliminary Study on the Early Apoptotic Peripheral Blood Lymphocytes in Patients with Systemic Lupus Erythematosus

摘要: 目的 通过对系统性红斑狼疮(SLE)患者外周血淋巴细胞(PBL)早期凋亡率测定,探讨其在SLE发病中的作用及相关性。方法 应用最新的Annexin Ⅴ试剂盒及流式细胞仪对SLE患者及年龄、性别相匹配的正常人PBL进行早期凋亡细胞的检测;并运用免疫印迹法检测SLE患者组自身抗体谱及应用免疫浊度法检测补体C3含量。结果 SLE活动期患者PBL早期凋亡率(13.46±4.13)明显高于稳定期患者(5.29±2.74)及正常人对照(5.14±2.33);稳定期患者与正常人对照差异无显著性(P>0.05);活动期患者中初发病组与已用皮质类固醇或免疫抑制剂治疗组差异无显著性(P>0.05)。SLE患者中,能测定出两种以上自身抗体阳性者凋亡率(13.44±4.40)明显高于至多有一种自身抗体阳性者凋亡率(6.72±3.86);且活动期患者PBL凋亡率与补体C3含量呈高度负相关(r=-0.86,P<0.01)。结论 SLE患者PBL凋亡率明显高于正常人,可能通过释放过量自身抗原的方式诱导一系列自身抗体的产生,从而参与SLE的发生。

Abstract: Objective To study the relationship between the apoptosis of lymphocytes and its significance in the pathogenesis of SLE.Methods Apoptosis was detected by Annexin V and flow cytometry(FCM),anti-ENA autoantibodies were detected with Western blot,and complement(C3)was tested quantitatively by turbidimetry.Results The percentage of the early apoptotic lymphocytes in the peripheral blood of active SLE patients was significantly higher than that of inactive SLE patients(P<0.001)and that of normal healthy controls(P<0.001),but there was no significant difference between quiescent SLE and normal healthy controls(P>0.05).Of the active SLE patients,no difference was found between those treated and not treated with corticosteroids or immunosuppressive agents(P>0.05).The percentage of apoptotic lymphocytes in those with more than two kinds of autoantibodies was markedly higher than those with only one kind of autoantibody(P<0.001)in SLE patitents.Besides,the rate of lymphocyte apoptosis was negatively correlated with serum complement C level in active SLE patients(r=-0.8587,P<0.01).Conclusion The increase of early apoptotic lymphocytes in the peripheral blood of SLE patients may contribute to the pathogenesis of SLE by producing excessive amount of autoantibodies.
