线性IgA大疱病基底膜带自身抗体的靶抗原异质性

Heterogeneity of Target Antigens of Basal Membrane Zone Antibodies in Linear IgA Bullous Dermatosis

摘要: 目的 了解线性IgA大疱病基底膜带自身抗体(BMZ-Ab)靶抗原及其与其它表皮下自身免疫性大疱病相关抗原的关系。方法 用热分离法提取真表皮抗原,免疫印迹检测11例线性IgA大疱病中IgA型和IgG型BMZ-Ab靶抗原的分布情况。结果 11例线性IgA大疱病患者中9例血清中IgA型BMZ-Ab可分别与真、表皮抗原结合,其中7例结合97000表皮抗原(LABD97),2例同时结合180000抗原,另有2例成人型线性IgA大疱病分别结合180000或230000表皮抗原。5例IgG型BMZ-Ab结合表皮97000、145000、180000、230000抗原,未见与真皮抗原发生反应。结论 线性IgA大疱病中BMZ-Ab的靶抗原表现为异质性,除LABD97以外,还可以是BP180、BP230等大疱性类天疱疮(BP)相关抗原。LABD97可能是BP180分子的降解产物,线性IgA大疱病是否是一种独立疾病应深入研究。

Abstract: Objective To detect the target antigens of basal membrane zone antibodies (BMZ-Ab) in linear IgA bullous dermatosis (LABD). Methods Dermal and epidermal antigens were extracted by heat-separated procedure, then the target antigens of BMZ-Abs in 11 LABD sera were detected. Results The IgA BMZ-Abs in 9/11 LABD sera could reacted with dermal and epidermal antigens respectively. 7/11 LABD sera reacted with 97 000 epidermal antigen (LABD 97), 2 sera reacted with 180 000 epidermal antigen simultaneously. 2 adult LABD sera recognized 180 000 and 230 000 epidermal antigen respectively. IgG BMZ-Ab in 5/11 LABD sera reacted with 97 000, 145 000, 180 000, 230 000 epidermal antigens, and did not react with dermal antigens. Conclusions The target antigens of IgA BMZ-Abs in LABD were heterogeneous including LABD 97, BP 180 and BP 230. LABD 97 may be a degraded fragment of BP 180. Whether LABD is an independent entity or not should be further investigated.
