女性寻常型痤疮患者黄体期八项性激素和皮质醇水平的检测

Study on Serum Levels of 8 Sex Hormones and Cortisol in the Luteal Phase of Menstrual Cycle in Women with Acne Vulgaris

摘要: 目的 探讨女性寻常型痤疮患者体内性激素和皮质醇水平的改变及与临床的关系。方法 采用放射免疫分析法,对54例女性寻常型痤疮患者黄体期血清中8种性激素及皮质醇水平进行了测定,并以30例正常女性作为对照。结果 患者组睾酮、雄烯二酮、硫酸脱氢表雄酮均显著高于对照组(P<0.01);雌二醇、孕酮、泌乳素、卵泡刺激素和黄体生成素无明显变化(P>0.05);皮质醇水平明显降低(P<0.05);月经前加重组雌二醇较正常人对照明显降低(P<0.05);<25岁年龄组雌二醇较正常人对照明显降低(P<0.05)。结论 雄激素升高可能是女性痤疮发生和加重的主要原因;肾上腺皮质参与了痤疮的发病;月经前期痤疮加重可能是因为黄体功能不全、雌二醇分泌较少所致;青春期时痤疮较重,而随年龄增长渐好转或自愈,可能与年轻时卵巢功能发育不全雌二醇分泌较少有关。

Abstract: Objective To investigate serum levels of 8 sex hormones and cortisol in women with acne vulgaris and their clinical significance. Methods The serum levels of 8 kinds of sex hormone and cortisol were measured with radioimmunoassay in the luteal phase of menstrual cycle in 54 women with acne, compared with 30 healthy controls. Results It was revealed that the levels of testosterone, androstenedione and dehydroepiandrosterone-sulfate were significantly higher in patients (3.30±2.76 nmol/L, 11.44±3.95 nmol/L, 6.79±3.88μ mol/L) than those in the controls (1.57±0.67 nmol/L, 8.66±1.47 nmol/L, 4.07±1.53μ mol/L) (P<0.01). No significant difference was found in the levels of estradiol (E2), progesterone,prolactin, follicle-stimulating hormone and luteinizing hormone between the two groups (P>0.05). The level of cortisol was significantly decreased in patients with acne (P<0.05). A significant fall of E2 level was observed in women with exacerbated acne before menses (P<0.05) and those who were younger than 25 years old (P<0.05). Conclusions The study indicates that the hyperandrogenaemia may be the major reason which causes and aggravates acne. The function of adrenal cortex may be associated with the pathogenesis of acne. Insufficiency of luteal function results in low level of E2, which may play a role in exacerbation of acne before menses. The decreased E2 due to immaturity of ovarian function could explain the tendency towards severe acne in young women.
