浓缩生长因子对改善皮瓣缺血再灌注损伤的临床疗效观察

Clinical efficacy of concentrated growth factor in ameliorating ischemia-reperfusion injury in skin flaps

摘要: 【摘要】 目的 观察浓缩生长因子对改善皮瓣缺血再灌注损伤的临床疗效。方法 收集2019年1月至2024年8月杭州市第三人民医院收治的16例手术后出现皮瓣缺血再灌注损伤的患者资料,男10例,女6例。根据缺血性皮瓣的部位分成两组,试验组8例,在皮瓣缺血部分多点注射浓缩生长因子,间距0.5 cm,每个点位注射约0.2 ml;对照组8例予以常规换药。观察皮瓣的颜色、质地及缺血面积,组间比较采用t检验。结果 试验组术前缺血面积为(406.6 ± 245.2) mm2,治疗后1周下降至(66.6 ± 80.6) mm2;治疗有效面积为(340.0 ± 263.9) mm2,治疗有效率为79.8% ± 26.4%。对照组8例术前缺血面积为(440.8 ± 208.4) mm2,治疗后1周下降至(346.9 ± 189.8) mm2;治疗有效面积为(93.9 ± 43.1) mm2,有效率为26.4% ± 18.2%;试验组治疗有效率显著高于对照组(t = 4.71,P = 0.001)。结论 浓缩生长因子可以改善患者术后皮瓣缺血再灌注损伤。

Abstract: 【Abstract】 Objective To evaluate the clinical efficacy of concentrated growth factor in improving skin flap ischemia-reperfusion injury. Methods Data were collected from 16 patients with ischemia-reperfusion injury in skin flaps after surgery at the Hangzhou Third People′s Hospital from January 2019 to August 2024. Ten males and 6 females were included. According to the sites of ischemic flaps, the 16 patients were divided into two groups: an experimental group (8 cases) receiving multi-point injections of concentrated growth factor at a dose of approximately 0.2 ml per point in the ischemic part of the flap at intervals of 0.5 cm; a control group (8 cases) receiving conventional dressing changes. The color, texture and ischemic area of the skin flaps were observed. The t test was used for comparisons between groups. Results In the experimental group, the preoperative ischemic area was 406.6 ± 245.2 mm2, which decreased to 66.6 ± 80.6 mm2 one week after treatment; the effective treatment area was 340.0 ± 263.9 mm2, resulting in a treatment response rate of 79.8% ± 26.4%. In the control group, the preoperative ischemic area was 440.8 ± 208.4 mm2, which decreased to 346.9 ± 189.8 mm2 one week after treatment; the effective treatment area was 93.9 ± 43.1 mm2, with a response rate of 26.4% ± 18.2%. The treatment response rate was significantly higher in the experimental group than in the control group (t = 4.71, P = 0.001). Conclusion Concentrated growth factor can improve postoperative ischemia-reperfusion injury in skin flaps.
