系统性红斑狼疮患者可溶性肿瘤坏死因子受体及肿瘤坏死因子α水平的研究

Levels of sTNFR、 TNFα in Serum and PBMC Culture Medium Supernatants of Systemic Lupus Erythematosus

摘要: 目的 了解系统性红斑狼疮(SLE)患者血清中可溶性肿瘤坏死因子受体(sTNFR)和肿瘤坏死因子α(TNFα)水平、两者的比值以及同临床症状的关系.方法 采用双抗体夹心ELISA法检测血清和外周血单一核细胞(PBMC)培养上清液中TNFα、sTNFRⅠ及sTNFRⅡ水平,同时检测其它免疫项目.结果 ①SLE患者血清中sTNFRⅠ、sTNFRⅡ、TNFα水平及sTNFRⅠ/TNFα和sTNFRⅡ/TNFα比值显著高于正常人对照组(P<0.01),sTNFRⅠ与sTNFRⅡ之间呈显著正相关(r=0.88,P<0.01).②PBMC培养上清液中TNFα、sTNFRⅠ及sTNFRⅡ水平显著高于正常人对照组,且与血清中的TNFα和sTNFR浓度呈显著正相关(r=0.58~0.73;P<0.01).③TNFα与感染有平行关系,而sTNFRⅠ和sTNFRⅡ同临床症状评分呈正相关(r值分别为0.69和0.75).结论 SLE患者sTNFR、TNFα水平及sTNFR/TNFα均有异常,sTNFRⅠ对封闭和抑制TNFα起作用,而sTNFRⅡ可能在协助sTNFRⅠ及炎症方面发挥作用,sTNFRⅠ及sTNFRⅡ对于监测SLE疾病活动和严重程度明显优于抗dsDNA抗体等指标.

Abstract: Objective To study the levels of soluble tumor necrosis factor receptor (sTNFR)、tumor necrosis factor alpha(TNFα)in systemic lupus erythematosus(SLE)patients and its clinical significance. Methods Double antibody sandwich ELISA was used to determine soluble tumor necrosis factor receptor typeⅠ (sTNFRⅠ)、soluble tumor necrosis factor receptor typeⅡ (sTNFRⅡ)and TNFα levels in patient serum and peripheral blood mononuclear cell(PBMC)culture supernatants. Other immunological tests were carried out simultaneously. Results ① The serum sTNFRⅠ、sTNFRⅡ、TNFα sTNFRⅠ/TNFα and sTNFRⅡ/TNFα ratios were significantly higher in SLE patients than those in normal control group(P<0.001).There was a significant positive correlation between the levels of sTNFRⅠ and sTNFRⅡ (r=0.88;P<0.001).② The levels of TNFα、sTNFRⅠ and sTNFRⅡ in SLE PBMC culture supernatant were significantly higher than those of normal controls with a positive correlation to the serum levels(r=0.58~0.73;P<0.001).③ The increased level of TNFα was related to infection,whereas the sTNFRⅠ、Ⅱ levels were positively correlated with the clinical scores. Conclusion There is an abnormal TNFα and sTNFR levels in SLE patients,which may play certain roles in the pathogenesis of SLE,and can be used as indices for active SLE.
