扁平疣临床表现不同皮损的病理观察

摘要： 目的 了解扁平疣的自愈现象的微观变化过程,探讨扁平疣自愈现象的可能诱因。方法 光镜观察105个扁平疣皮损标本。结果 扁平疣稳定状态下皮损的病理表现为角化过度,棘层肥厚,表皮突轻度伸长及空泡变性。进行性增多时的皮损以塔峰状表皮角化、楔型粒层增厚、空泡化细胞三者的同时出现为特征;发红、发痒时皮损的病理表现以表皮内灶性和弥漫性淋巴细胞浸润、角质形成细胞坏死为特征。结论 扁平疣的自身免疫过程和淋巴细胞表皮内浸润有关,皮损增多,特别是急剧地增多似乎能诱导自身免疫的发生。

Abstract: Objective To find out the pathologic changes and possible influencing factors in the process of spontaneous regression of plane warts.Methods One hundred and five lesional specimens taken from patients with plane warts were observed microscopically.Results The histologic features indicated slight hyperkeratosis,acanthosis and ballooning degeneration of keratinocytes in stable lesions;a combination of tower-peak shaped hyperkeratosis,cuneate hypergranulosis and ballooning degeneration in progressively increasing lesions;and infiltration of lymphocytes and necrosis of keratinocytes in inflammatory icthing lesions.Conclusions Specific cell-mediated immunity against keratimocytes may be involved in the process of regression of plane warts.There is a positive correlation between the activity of skin lesions and the induction of cellular immunity.
