同一患者两处脂溢性角化病皮损伴Meyerson 现象一例

Meyerson phenomenon around two lesions of seborrheic keratosis in a patient

摘要:

报道脂溢性角化病伴发Meyerson现象一例。患者男,65岁,因左上背部及左小腿褐色丘疹20余年,周边发生红斑、丘疹、渗出伴瘙痒2月就诊。专科检查:左侧小腿及左上背部各有一褐色丘疹,0.5cm大小,周围可见1-1.5cm直径范围内分布红斑、丘疹、鳞屑,伴有轻度渗出,边界较清,中央褐色丘疹边缘规则,无溃疡、渗血。两处皮损组织病理学检查基本一致:1、中央褐色丘疹:棘层增生肥厚伴水肿,棘层内可见假性角囊肿,增生表皮底部平坦,真皮浅层血管周围可见慢性炎性细胞浸润。2. 丘疹边缘部湿疹样改变处:表皮角化过度伴角化不全,棘层肥厚,海绵水肿,真皮浅层及血管周围有淋巴细胞、组织细胞和少量嗜酸性粒细胞浸润。诊断:脂溢性角化病伴Meyerson现象

Abstract:

one case of seborrheic keratosis accompanied with Meyerson phenomenon was reported. A 65-year-old male presented with brown papules on his left upper back and left leg for more than 20 years and erythema, papules and exudation around the original brown papules with itchy for more than 2 months. Physical examination showed that there were one 0.5cm in diameter sized brown papules on his left leg and left upper back respectively. Each papule was surrounded by a 1-1.5cm in diameter circumscribed erythema accompanied with papules, scales and mild exudation. The brown papules in the center had a clear border and had no ulceration or bleeding. The histopathological changes of both lesions from the back and left leg were similar: the brown papules from the center of the lesions showed acanthosis with edema, pseudokeratotic cysts in and a flattened base of the epidermis, perivascular chronic inflammatory infiltration in the superficial dermis. The samples from the surrounding eczematous changes of the brown papules showed hyperkeratosis, parakeratosis, acanthosis, spongiosis of the epidermis, an infiltration composed of lymphocytes, histiocytes and a few eosinophils in the dermis and perivascularly. The final diagnosis was seborrheic keratosis accompanied with Meyerson phenomenon.
