532 nm皮秒激光治疗面部早期脂溢性角化病的疗效和安全性分析

Analysis of efficacy and safety of 532-nm picosecond laser in the treatment of early-stage facial seborrheic keratosis

摘要: 【摘要】 目的 评价532 nm皮秒激光治疗面部早期脂溢性角化病的疗效和安全性。方法 前瞻性选择2020年12月至2022年9月在宁夏医科大学总医院皮肤科门诊就诊的95例面部早期脂溢性角化病患者,均采用532 nm皮秒激光治疗1次,并随访6个月。采用4分法整体评估皮损消退情况以评价临床疗效,采用VISIA皮肤检测仪定量检测治疗前后棕色斑的特征计数、绝对分值和百分位数,采用配对样本t检验比较治疗前后棕色斑定量指标的变化。评估患者的疼痛等级,记录不良反应的发生情况。结果 95例面部早期脂溢性角化病患者经532 nm皮秒激光治疗1次后,均完成6个月的随访。所有患者治疗区域皮损消退均达25%以上,10例疗效一般,17例疗效良好,68例疗效显著,治疗有效率为89.47%(85/95)。VISIA皮肤检测仪检测结果显示,治疗后6个月棕色斑的特征计数[(195.19 ± 51.06)个]和绝对分值[(28.80 ± 6.20)分]均显著低于治疗前[(211.48 ± 50.94)个、(35.16 ± 6.84)分,t = 12.73、16.90,均P < 0.001],而棕色斑的百分位数显著高于治疗前(38.48% ± 10.80%比30.61% ± 10.27%,t = -15.73,P < 0.001)。所有患者在治疗过程中均有不同程度疼痛,但均可耐受并坚持完成治疗,疼痛评估分值为2 ~ 6(3.64 ± 1.67)分。9例(9.47%)出现暂时性炎症后色素沉着斑,4例(4.21%)出现暂时性炎症后色素减退斑,随访6个月内,均恢复至正常肤色。结论 532 nm皮秒激光治疗面部早期脂溢性角化病安全、有效。

Abstract: 【Abstract】 Objective To evaluate the efficacy and safety of 532-nm picosecond laser in the treatment of early-stage facial seborrheic keratosis. Methods A total of 95 patients with early-stage facial seborrheic keratosis were prospectively enrolled from the Department of Dermatology, General Hospital of Ningxia Medical University between December 2020 and September 2022. All the patients received a session of 532-nm picosecond laser treatment, and were followed up for 6 months. A 4-point scale was used to evaluate the overall improvement of skin lesions for assessing the clinical efficacy. A VISIA skin detector to quantitatively determine the characteristic counts, absolute scores, and percentiles of brown spots before and after treatment, and the paired sample t-test was used to compare the quantitative indicators of brown spots before and after treatment. The patients′ pain grades were evaluated, and adverse reactions were recorded. Results All the 95 patients with early-stage facial seborrheic keratosis received a session of 532-nm picosecond laser treatment, and completed a 6-month follow-up. All the patients achieved over 25% regression of skin lesions in the treatment area, of whom 10 received mild improvement, 17 received favorable improvement, and 68 received marked improvement, with the response rate being 89.47% (85/95). The results of the VISIA skin detector showed that the characteristic counts (195.19 ± 51.06) and absolute scores (28.80 ± 6.20 points) of brown spots significantly decreased and the percentiles of brown spots (38.48% ± 10.80%) significantly increased at 6 months after treatment compared with the corresponding baseline indicators (211.48 ± 50.94, 35.16 ± 6.84 points, 30.61% ± 10.27%, t = 12.73, 16.90, -15.73, all P < 0.001, respectively). All the patients experienced varying degrees of pain during the treatment, with the pain scores being 2 - 6 (3.64 ± 1.67) points, but all of them could tolerate the pain and completed the treatment. Temporary postinflammatory hyperpigmentation and hypopigmentation occurred in 9 (9.47%) and 4 (4.21%) patients respectively, which recovered to normal in color during the 6-month follow-up. Conclusion The 532-nm picosecond laser was safe and effective for the treatment of early-stage facial seborrheic keratosis.
