角化棘皮瘤与人乳头瘤病毒相关性研究

A study on the relationship between human papillomavirus infection and keratoacanthoma

摘要: 目的 探讨角化棘皮瘤组织中HPV的感染率,观察HPV DNA阳性细胞的分布特征.方法 应用原位杂交方法检测26例角化棘皮瘤和26例正常人皮肤石蜡切片组织标本中HPV DNA.结果 10例角化棘皮瘤HPV DNA阳性,阳性率38.46%(10/26);阳性细胞多位于火山口样凹陷表皮下方增生的棘细胞层;病例组HPV DNA阳性与阴性患者在性别、发病年龄、病期以及皮损大小的差异无统计学意义;病例组与对照组HPV DNA阳性率经x2检验,差异有统计学意义(P<0.01).结论 角化棘皮瘤患者中HPV感染率高于正常人群,但HPV感染与角化棘皮瘤患者发病年龄、性别、病期和皮损大小无关.

Abstract: Objective To evaluate the infection rate of HPV in keratoacanthoma(KA)patients, and to observe the distribution of HPV DNA positive cells in KA tissues.Methods Tissue specimens were resected during operation from 26 KA patients(diagnosed with clinical and histopathological manifestadons)and 26 normal controls, then embedded in paraffin and subjected to in situ hybridization (ISH) for the examination of HPV DNA.Results HPV DNA was detected in 10(38.46%, 10/26)patients, but not in any of the controls, with a significant difference between the two groups in the detection rate(P<0.01).Of the 10 patients positive for: HPV DNA, 9 presented with lesions of mature stage, and 1 with lesions of regressive stage, as shown by the histopathological examination.HPV DNA positive cells were predominately located in the hyperplastic stratum spinosum under crater-shaped epidermal depression, and most of the cells aggregated together.The differences were of no statistical significance in gender, onset age, disease duration or tumor size between HPV DN,A negative and positive patients(all P >0.05).Conclusions Compared with the normal population, KA patients have a higher infection rate of HPV, and the infection appears to be unrelated to the age, gender, disease duration or tumor size of these patients.
